Abstract

Very Early-Onset Inflammatory Bowel Disease (VEO-IBD) refers to inflammatory bowel disease with onset before the age of 6, exhibiting significant differences in clinical manifestations, genetic background, and treatment response compared to late-onset IBD. Research indicates that host monogenic defects play a key role in the pathogenesis of VEO-IBD, with mutations in IL-10 and its receptor (IL-10R) genes being particularly important. This article systematically elaborates on the pathogenic mechanisms, clinical features, diagnostic strategies, and therapeutic advancements related to the IL-10/IL-10RA signaling pathway, aiming to provide a theoretical basis for the development and application of relevant detection kits.

I. Overview and Genetic Background of VEO-IBD

VEO-IBD accounts for 5%–15% of pediatric IBD cases, with an incidence rate of approximately 4.37 per 100,000. Perianal lesions are prominent, manifesting as multiple anal fissures, abscesses, or fistulas. Endoscopy reveals widespread skip ulcers and cobblestone-like changes in the colon (Figure 2). Histopathology is characterized by crypt abscesses, intraepithelial granulomas, and dense mononuclear cell infiltration (Figure 3). Some patients exhibit extraintestinal manifestations such as recurrent infections, folliculitis, or eczema (Figure 4), indicating systemic immune dysregulation.

IV. Diagnostic Strategies and Application of Genetic Testing

Diagnosis of VEO-IBD requires integrating clinical, endoscopic, pathological, and genetic evidence. For children with onset before age 6, severe disease, significant perianal lesions, poor treatment response, or a family history, monogenic mutations should be highly suspected. Genetic testing for IL-10/IL-10RA and related genes can identify pathogenic mutations, providing a basis for precise classification and treatment.

V. Current Treatments and Transplant Interventions

Conventional anti-inflammatory and immunosuppressive therapies have limited efficacy in children with IL-10 pathway defects. Nutritional support and surgical interventions may temporarily alleviate symptoms but often come with healing difficulties and recurrence risks. Currently, allogeneic hematopoietic stem cell transplantation has been shown to induce clinical remission in some children with IL-10/IL-10RA gene mutations, offering a potential cure.
